Pfizer Inc. respects your privacy. This Privacy Notice describes the practices in connection with the information collected for the Acute Respiratory Illness Surveillance research study or AcRIS research study (the “AcRIS Study”) through the study’s mobile app, any associated software, www.brainbaseline.com website, and any associated documentation (collectively, the “App”) hosted and supported by our vendor, Digital Artefacts, L.L.C. (“DA”). This Privacy Notice covers the information that will be required to determine your potential eligibility to participate in the AcRIS Study and to confirm your identity, prior to you signing the informed consent. After you have signed the informed consent, you will be asked to provide information to determine your eligibility to participate in the AcRIS Study, and information for the study site to be able to report tests results in the event that you become eligible for and participate in the AcRIS Study. These steps will occur before you receive the signed informed consent back from the study team. If you are eligible and have been enrolled into the study, additional information relating to the treatment of your personal information collected in connection with the study will be governed by the Informed Consent Document (“ICD”). By using the App, you agree to the terms of this Privacy Notice. Please be advised that the App is hosted on BrainBaseline. If you are a consumer and obtained the App via iTunes, or Google Play please be aware that the App is operated by DA. This Privacy Notice governs the use and disclosure of information collected through the App. Please read this Privacy Notice carefully. Using the App is voluntary, and by using or accessing the App in any way, or by transmitting information to us by e-mail or other electronic means, you acknowledge that you have read and understood this Privacy Notice, and that your access and use of the App are subject to this Privacy Notice as well as the terms and conditions of the App. Your continued use of the App will constitute your acceptance of the terms of this Privacy Notice. If you do not agree with the terms of this Privacy Notice, please do not access or use the App.

INFORMATION YOU PROVIDE

The App collects the personal information you choose to provide (“User Information”).

The app may ask for the following User Information when you use the App, including:

Personal Information (PI) such as email, home address, shipping address, first and last name, last 4 digits of Social Security Number (SSN), and date of birth (DOB) for identity verification and for use by the study team to report the PCR tests results to CDC and/or local/state health authorities
Personal Information (PI) such as your name, email address, and COVID test results, so that you can be offered the opportunity to be informed about other ongoing or future Pfizer studies. If you agree to receive such study information, your personal information, will be shared with a vendor contracted by Pfizer in order for that vendor to send you information on other Pfizer studies that you, or members of your household, may be interested in.
Study specific questionnaires and pre-screening related to but not limited to medical history and self-reported information for eligibility criteria
Demographic information (sex, race, ethnicity, residence zip code, residence county and country) for use by the investigator to report the PCR tests results to CDC and/or local/state health authorities
Phone number with area code as a contact method for your potential participation in the study
Optional Emergency Contact Information as an alternative contact method for your potential participation in the study
Usage Data (purely structured, technical and automated data)

HOW WE USE AND DISCLOSE THE INFORMATION YOU PROVIDE

We will not collect, store, analyze, disclose, or otherwise use this User Information except as necessary to: process data you have sent us; respond to your queries or requests; provide you with technical support or updates; act in a manner consistent with an informed consent document you signed, if any; or enforce the terms of this Privacy Notice, Terms and Conditions, and any other policies or legal requirements governing the use of the App. In submitting this User Information to us, you consent to our use of it for these limited purposes.
We may use User Information (other than data stored on the device to which Pfizer and its vendor, DA have no access) to:

Confirm your eligibility in the study and meet study requirements
Improve the App’s functionality and provide customer service to you.
Respond to your inquiries and fulfill your requests, such as to send you notifications and password resets.
Send administrative information to you, such as information regarding the App and changes to our terms.
Send you information about other Pfizer Studies, if you agree to receive information on other Pfizer Studies.

We may disclose User Information (including data stored on device to which Pfizer and its vendor, DA have no access) to:

Our affiliates, for the purposes described in this Privacy Notice.
Our third-party service providers, to permit them to provide authorized services on our behalf, such as data management and analysis; identity verification, eligibility/screening, information technology and related infrastructure provisioning; customer service; email delivery; auditing; and other approved services, including, recruitment for other Pfizer studies.
A third party, in the event of any reorganization, merger, sale, joint venture, assignment, transfer or other disposition of all or any portion of our business, assets or stock (including in connection with any bankruptcy or similar proceedings).

We may also use and disclose the information you provide, as we believe to be necessary or appropriate to:

Comply with applicable law, including laws outside your country of residence.
Comply with legal process.
Respond to requests from public and government authorities, including authorities outside your country of residence.
Enforce our terms and conditions.
Protect our operations, or those of any of our affiliates.
Protect our rights, privacy, safety or property, and/or that of our affiliates, you or others.
Allow us to pursue available remedies or limit the damages that we may sustain.

We may aggregate the information that you and other App users provide on a de-identified basis. If we do, we may use and disclose such aggregated information for any purpose. Aggregated information does not personally identify you or any other user.

DATA STORAGE AND ENCRYPTION

Your User Information is stored and encrypted as described in this section.
A. The following data is always transferred to and stored on an external cloud server (encrypted data marked with *):

Name (first name and last name) and email address, date of birth (DOB), home address, shipping address, social security number (SSN), and Password*
Study questionnaires (including pre-screening and screening)

If you choose to register with the App, the personal information you provide as part of your registration for/opt-in to the App will be used by Pfizer and its vendor, DA, and any other third-party service providers providing services on Pfizer’s behalf, in accordance with this Privacy Notice.

OTHER INFORMATION

We may use cookies and other technologies to recognize you as you use or return to the App so that we can understand how the App is used and personalize and enhance your experience. These technologies collect certain information automatically, such as:

Information about your device, such as your screen resolution, operating system, device manufacturer and model, language and browser type. We use this information to ensure that the App functions properly.
Information about your use of the App, including content viewed, features used and the dates and times of your interactions with the App on an aggregate and de-identified basis. We use this information to understand how users engage with the App and to customize and improve the App experience.

DO NOT TRACK

Do Not Track ("DNT") is a privacy preference that users can set in their web browsers. When a user turns on DNT, the browser sends a message to websites requesting that they not track the user. At this time, Pfizer and DA do not respond to these signals. For information about DNT, visit www.allaboutdnt.org.

THIRD-PARTY SERVICES

You agree that you have provided notice to, and obtained consent from, any third party individuals whose Personal Information you supply to us, including with regard to: (a) the purposes for which such third party’s Personal Information has been collected; (b) the intended recipients or categories of recipients of the third party’s Personal Information; (c) which of the third party’s Personal Information is obligatory and which Personal Information, if any, is voluntary; and (d) how the third party can access and, if necessary, correct the Personal Information provided.
This Privacy Notice does not address, and we are not responsible for, the information collection, use, disclosure or security practices, or other practices, of any third party, including any third party operating a service to which the App links. The inclusion of a link within the App does not imply our endorsement of the linked service.

INFORMATION SECURITY

We seek to use reasonable organizational, technical and administrative measures to protect the information you provide to us. Unfortunately, no data transmission or storage system can be guaranteed to be 100% secure. If you have reason to believe that your interaction with us is no longer secure, please immediately notify us.

YOUR PRIVACY CONTROLS

You may stop all collection of information by the App by uninstalling the App. If applicable, you may stop the receipt of push notifications through your device settings.

USE BY MINORS

Participation in the AcRIS Study in restricted to individuals eighteen (18) and older. The App is not directed to individuals under the age of eighteen (18), and we do not knowingly collect personal information from such individuals.

CROSS-BORDER TRANSFER

The App is controlled and operated by us from the United States and is not intended to subject us to the laws or jurisdiction of any state, country or territory other than that of the United States. The information we collect through the App may be stored and processed in any country where we have facilities or in which we engage service providers, and by using the App, you consent to the transfer of information to countries outside of your country of residence, including the United States, which may have different data protection rules than those of your country.

UPDATES

From time to time, we may update this Privacy Notice. Any changes will become effective when we post the revised Privacy Notice in the App. Your use of the App following these changes means that you accept the revised Privacy Notice. This Privacy Notice was last updated as of the “Last Updated” date shown above.
